on 12th January 2007:

• BREAKOUT Application Form (3 copies)
• Monitoring Form (1 copy)
• 1 page outlines for (up to) 3 potential project ideas
• (3 copies x each idea)
• Examples of previous work, as stipulated in the
• BREAKOUT Guidelines

NB: Applications CANNOT be submitted electronically.
